If you have requested your records and it has been 30 days PLEASE resend your request. We are trying to do the best we can with the limited resources we have. Gmail accounts and bank assigned emails will not accept these files.

With many months of thoughts, tears, and sadness, my staff and I would like to inform the patients, and you, whom we love that we must close after the last appointment on June 9th 2014. The practice will be filing Bankruptcy. A selected trustee will be given the list of credits and balances for future financial management and collections. Until then we look forward to providing care for hygiene and dental appointments as usual.
The economic changes throughout the years have caused Drake Dentistry of Charlotte to close. I have gone to work each and every day enjoying every moment with patients and staff. All of the staff at Drake Dentistry of Charlotte will continue to work together until we have taken care of all patients. Sadly, my future is different, but promising with new changes and experiences. I will no longer be located in the area, but I would be happy to help you find a dentist to serve you and your family’s needs for the future.
I would like to make it very clear that all of your patient information is secure. All of our paper files are being stored with a company called File Solve. File Solve is a HIPPA compliant document storage company that is knowledgeable in the healthcare industry and in healthcare document management. Also, all electronic files will be in the hands of Solution Start, our IT provider, and myself. I will be in direct control of all patient records until a suitable dentist is found to manage these records. Information will be sent to all patients notifying them of the selected dentist. If you need your records before a new dentist is selected, please download the form from the website. Email or fax the completed form to staff as directed. (These forms can be faxed to 704-496-2163 or emailed to firstname.lastname@example.org.) NO patient information will be left in our office.
For the next 30 days the office staff and I will be available to answer your questions and address your concerns. Please feel free to call the office at 704-338-9797 and leave a message or email your questions and concerns to email@example.com. I will be working with the staff to address patient questions, but the expected volume will make it difficult to address each individual personally. Due to the nature of the office being closed, an email is the preferred way of communication. Emails will be answered in the order received. Patients in pain must be given preference and will be answered first.

The office staff and I will respond to your call and help you find the appropriate care for your situation. The office staff and I will work with you and assist you in finding a referral to a specialist or the emergency dental clinic.
For patients whose treatment has not been completed, we will help you find a dentist of your choosing to complete your care. For our ClearCorrect patients, we will help you find another dentist of your choosing that can complete your treatment. Within a 20 mile radius of our office there are over 60 ClearCorrect providers, and 22 of these are within a five mile radius. Per ClearCorrect these dentists must honor your treatment and they CANNOT charge you again for an active ClearCorrect case. The only fee that can be charged to you is for an office visit. Please go to http://clearcorrect.com/how/FindADoctor.aspx and type in your zip code to find a ClearCorrect dentist of your choosing. Also, you can go to our website and download the PDF with a list on ClearCorrect provider within a 20 mile radius of our office. You must download, sign, and return the records transfer form from our website and indicate where you would like your ClearCorrect case transferred.
